Patient Forms

For your convenience, we have provided some common patient forms in Adobe PDF format for you to download and complete prior to your visit with us. You may also email the forms back to Henderson & Walton, or print them for later use.

IMPORTANT: Form Email Instructions

Because of the ways different web browsers manage PDF files, email functionality is available only through the use of Adobe Acrobat Reader software. To use a form’s email functionality, follow the steps listed below:

  1. Click on the desired link to download the form to your computer. If prompted to either open or save the file, please choose save. If the browser offers to open the file after download, click cancel.
  2. Exit the browser and navigate to the downloaded form on your computer.
  3. Double-click on the file icon, and the form should open in Acrobat Reader. If Acrobat Reader is not installed on your computer, click here for a free download and installation instructions.
  4. Complete and save the form, then click the Send button at the bottom of the form. The form will then show up as an attachment to an email addressed to Henderson & Walton.

You may also complete, save, and print a hard copy of the form to bring with you to your appointment.
